SEND Information

At Chapelfield Primary School, we are committed to ensuring every pupil can achieve their potential. Our approach to special educational needs (SEN) is rooted in our vision to equip children with the skills, knowledge and attitudes for life beyond school and in our values of Honesty, Respect, Caring and Commitment. We work closely with families and local partners to identify needs early, provide timely support and celebrate progress.

How we identify and assess needs

Teachers and support staff are trained to notice when a pupil needs extra help. Needs may be identified through:

    • day‑to‑day classroom observation and formative assessment
    • progress tracking and termly pupil progress reviews
    • information from parents, early years settings or previous schools
    • screening and targeted assessment (for example reading, speech and language or social communication)

When a need is identified the class teacher will:

    • discuss concerns with parents/carers and the pupil
    • agree clear, measurable outcomes and a plan of support
    • put targeted classroom strategies and short‑term interventions in place

We follow the Special Educational Needs and Disability (SEND) Code of Practice. Where progress remains limited despite well‑targeted support, we will consider additional specialist assessment or advice.

Our graduated approach to support

We use a graduated, personalised approach:

  • High‑quality classroom teaching that is inclusive and adapted to meet a range of needs.
  • Targeted interventions delivered in class or in small groups, with clear objectives and regular review.
  • Specialist support from external services (for example, Educational Psychologists, Speech and Language Therapy or the Additional Needs Team) when required.

Education, Health and Care (EHC) needs assessments are considered where a pupil's needs are complex and long‑term. Parents are fully involved at every stage.

 What we provide

Through our Ordinarily Available Inclusive Provision we provide
  • Inclusive classroom practice and reasonable adjustments to the learning environment
  • Differentiated curriculum and resources within our cyclical Curriculum (Cycle A & Cycle B) to meet individual needs
  • Small‑group and 1:1 interventions as indicated
  • Accessibility across our single‑storey building and a disabled accessible toilet
  • Support for pupils with medical needs in line with statutory duties
  • Trained and nurturing staff
  • A nurture informed approach to behaviour management

These arrangements reflect our commitment to treat pupils as individuals and to provide a secure, stimulating environment where learning is enjoyable and challenging.

Partnership with parents and pupils is central to our practice:

  • parents are communicated with regularly and invited to contribute to targets and reviews
  • pupils’ views are listened to and taken into account when planning support and outcomes
  • we provide clear information about progress and how parents can support learning at home

Inclusion and Participation

We aim for pupils with SEN to have full access to school life:

  • all school trips and clubs are risk‑assessed and reasonable adjustments are made so pupils can participate
  • extra pastoral support and listening opportunities are available for pupils needing help with emotional and social development
  • we celebrate achievement and encourage confidence and resilience in all pupils.

Admissions and Transition

  • Admission arrangements for pupils with SEN are the same as for all pupils. Where a pupil has an EHC plan, this is taken into account during the admissions process.
  • We make careful transition arrangements when pupils move between year groups, phases or to secondary school. Transition planning includes visits, sharing of information and, where appropriate, multi‑agency meetings to ensure continuity of support. We have a phased approach to transition which provides different levels of support dependent on a pupils needs. This is communicated to parents via letter in the summer term.

Evaluating effectiveness

  • We monitor and evaluate SEN provision through pupil progress reviews, provision maps, data analysis on the impact on intervention and regular SENCo and governor oversight.
  • Data on levels and types of need is shared with the local authority to support strategic planning and to ensure our offer remains effective and responsive.

SENCo: Mrs V Hands — responsible for coordinating SEN provision, liaising with parents and specialists, and reviewing progress.

Family Support Officer: Mrs J McLoughlin — helps families access services and supports wellbeing and attendance.

Headteacher: Mrs A Reed — overall responsibility for the school’s inclusive practice.

How to raise concerns or make a complaint

  • In the first instance speak to your child’s class teacher.
  • If concerns remain, contact the SENCo (Mrs V Hands) or the Headteacher (Mrs A Reed).
  • Our complaints procedure is available in the school policy documents; we encourage parents to raise concerns informally first so we can work together to resolve issues.
